After Dean beat the crap out of Sam at the end of last week's episode, he calls on Castiel for help. He discovers that Sam has no soul. So the boys need to get it back but noone knows how. Their only clue is that Samuel, their grandfather, was brought back at the same time.
They visit and discover that he is hunting the Alpha Vampire, the first one. They then discover that Samuel is torturing him for information on where Purgotry is. Of course he won't tell and he escapes. We then find out that Samuel is doing this work for the demon Crowley and it was he that brought Sam and Samuel back and it is he that has Sam's soul. So now the boys have to do whatever he says until they can figure out a way to get Sam's soul back.
Lots of story in this episode, setting up for the rest of the season.
